I do hereby enroll my child/myself to The Ballet School of Colombo (TBSC) and do hereby agree to abide by the Rules of the School set out below:

[a] I understand that participation in dance classes can and may result in possible personal injury to my child/myself despite all precautions being taken by TBSC and all its teachers, dancers and staff members. By ticking the box hereto I assume all risk related to any and all activity within and outside TBSC before, during or after class.

[b] I agree to release from responsibility TBSC including all teachers, dancers and staff members from any liability for any personal injury to my child/myself whether caused as a result of dancing or otherwise, including but not limited to; scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, demise or damage or loss to any personal property, that may occur on the premises before, during or after class.

[c] I voluntarily agree to take full responsibility for the conduct and safety of my child/myself before and after class and when outside the premise of TBSC and I agree to release all teachers, dancers and staff members from any responsibility for the conduct and safety of my child/myself before and after class or outside the premises of TBSC.

[d] I understand that payment in full is required for access to classes and agree to make payment at the beginning of each term. There will be three terms each year comprising of approximately a period of 3 1/2 months. I understand that failure to make payment will result in the denial of admission to classes.

[e] Once fees is paid for the allocated program it is nonrefundable. The payment made is purely for the intended term/month and can only be transferred on special request and pending approval from the admin team.

[f] Students of TBSC may be photographed and videotaped from time to time and by ticking the box below, I release TBSC from all liability of use of this material.
